Since version PDF extension-1.2.0.9-SNAPSHOT, the problem has occurred and is not present in 1.2.0.8-SNAPSHOT and earlier versions.

in which version was this issue indroduced why did the test case not catch this?

ah, might need to drop a new verison?
When I used the PDF extension 1.2.0.9-SNAPSHOT, it threw the error. However, when I run the same code on PDF extension 1.2.0.8-SNAPSHOT and below versions, it worked fine and didn't throw the error.
